Abstract

The integration of Artificial Intelligence (AI) into Clinical Decision Support Systems (CDSS) is transforming healthcare delivery by enhancing diagnostic accuracy, treatment planning, and patient outcomes. In developing countries, where healthcare systems often face constraints such as limited workforce capacity, inadequate infrastructure, and uneven access to specialist care, AI-driven CDSS presents significant opportunities for improving efficiency and bridging critical service gaps. This study examines the potential benefits and ethical challenges associated with integrating AI into CDSS within resource-constrained settings. Using a narrative review approach, relevant empirical and theoretical literature published between 2015 and 2025 was analysed to identify key trends, applications, and concerns.

Findings indicate that AI-enabled CDSS can support early disease detection, optimise clinical workflows, and enhance evidence-based decision-making, particularly in underserved and rural communities. However, several ethical issues persist, including data privacy and security risks, algorithmic bias arising from non-representative datasets, lack of transparency in AI decision-making processes, and limited regulatory frameworks in many developing countries. Additionally, disparities in digital literacy and infrastructural readiness may exacerbate health inequities if not adequately addressed.

The study underscores the need for context-sensitive implementation strategies that prioritise ethical governance, stakeholder engagement, and capacity building. It recommends the development of robust regulatory policies, inclusive data practices, and continuous training for healthcare professionals to ensure responsible AI adoption. Integrating ethical considerations into system design and deployment is essential to maximise the benefits of AI-driven CDSS while safeguarding patient rights and promoting equitable healthcare outcomes in developing countries.
